- Ko Runes could be one of the extra Runes that the Countess drops on Hell difficulty.
- A 1 in 11 chance of dropping a Ko Rune is one of the benefits of playing Hellforge on Nightmare or Hell difficulties.

Horadric Recipes
- Three Ko runes plus a defective sapphire can be transmuted to generate one Fal rune. (Only in ladder, single-player, or open formats).
- You can transmute an exceptional unique armor, a Ko rune, a Lem rune, and a Perfect Diamond to upgrade the base weapon type of unique armor to its elite variant. (open, ladder, or exclusive to one player)
- Transmute an extraordinary rare armor piece along with a Ko, Pul, and Perfect Amethyst rune to upgrade it from its base armor type to its elite variation.
- A Perfect Diamond, Ko, Lem, and exceptional set armor can be transmuted together with the base weapon type of an exceptional set armor to upgrade it to its elite variation.
